Payment Information
Simplify Your Billing and Payment Experience
The Good Faith Estimate provides typical charges for services at ONE Health Ohio and RISE Recovery for uninsured or underinsured patients not eligible for our Sliding Fee Scale discounts. These amounts are estimates and not a contract. Final costs may vary.
This Good Faith Estimate provides a cost range for visits with Medical, Dental, or Behavioral Health providers. It excludes unknown or unexpected costs, lab services, and additional procedures like dental x-rays. Charges will be reduced for patients who qualify for our Sliding Fee Scale discounts.
Sliding Fee Scale
Uninsured patients and those with high deductibles or copays may apply for SFS to receive discounted charges based on household income and size. Complete and submit an application with proof of income for approval. Verification is updated annually. Without proof of income, patients are responsible for 100% of charges. Qualified individuals pay a portion of the bill, ranging from a nominal fee to 100% of charges.
Private and Commercial Insurance
Patients must verify their network coverage. They are responsible for 100% of co-pays and deductibles. The sliding fee scale may apply to those with high deductibles or co-pays, if they qualify. If ONE Health Ohio is out of network, patients are responsible for 100% of all costs incurred. Prescriptions, referrals, and hospitalizations may not be covered if non-network providers are used. Payment for any non-covered services is the patient’s responsibility. For major medical insurance (hospitalization only) or services not covered (e.g., well child or dental), patients may apply for the sliding fee scale.
Medicaid
Covers all services provided by ONE Health Ohio. All Medicaid HMOs are accepted.
Ohio Disability/General Assistance
Covers medical services. For dental, it only covers tooth extractions and related x-rays. For other dental services, patients are eligible for the sliding fee scale with proof of income.
QMB/SLMB
These programs help pay medical premiums but are not medical cards.
Family Planning Cards
Only cover family planning services.
Medicare
Medicare covers medical services and flu shots for those 65 and older or on SSI. It does not cover dental services, SLMB, QI1/QI2, or QMB. However, Medicare patients can apply for the sliding fee scale for primary dental work.
20% of office visit co-insurance is billable to secondary insurance if applicable. Please inform us if you are enrolled in a Medicare Advantage Plan.
QMB Medicaid for Medicare patients does not cover dental services, but Medicaid pays medical office visit co-pays. Coverage is also available through Medicare Replacement Plans (Part C), such as Anthem Senior Advantage and UHC Dual Complete.
Payment Policies
Please present your Medicare, Medicaid, or insurance card at every visit to verify healthcare coverage before seeing a provider.
ONE Health Ohio is not a free clinic. We offer a sliding fee scale to assist the medically uninsured with healthcare costs.
Payment is required at the time of your visit, and you are responsible for any remaining balances. Financial arrangements can be made; please check with a Front Desk Receptionist for payment details at the time of your visit.
We accept cash and personal checks (with imprinted name and address). Postdated checks are not accepted. Most major credit and debit cards are also accepted. To pay by credit card, please visit the nearest ONE Health Ohio center or contact our billing department at 330-747-9551 ext. 10606.
