What documents should I bring to my first appointment?
Please bring a valid photo ID, insurance card, and any medical records or medications you are currently taking. If you are applying for the sliding fee scale, bring proof of income (recent pay stubs, tax returns) and family size.
What should I do if I need to cancel an appointment?
How do I sign up for the patient portal?
To sign up for the patient portal, visit our website and click on the ‘Patient Portal’ link. Follow the instructions to create an account using your email address and a secure password.
How do I pay for services?
We accept cash, personal checks (with an imprinted name and address), and most major credit and debit cards. Please note that postdated checks are not accepted. To pay by credit card, visit your nearest ONE Health Ohio center or contact our billing department at 330-747-9551 ext. 10606. There is financial assistance if needed!
Do you provide pharmacy services?
Yes, ONE Health Ohio operates an in-house pharmacy called the Prescription Pad at Falls Family Care, Youngstown, Lloyd McCoy, and Warren West locations. It is available to all ONE Health Ohio and RISE Recovery Patients where they offer discounted medications and coordinate care with your medical clinician.
