Choose Your Protection: Pfizer and Moderna Vaccines Available
Pfizer COVID-19 Vaccine and Booster
The Pfizer COVID-19 vaccine and booster are available at two ONE Health Ohio health centers for patients aged 12 and older. Vaccinations are by appointment only at:
Youngstown Community Health Center: 726 Wick Ave., Youngstown, OH 44505
Lloyd McCoy Community Health Center: 1977 Niles Rd. SE, Warren, OH 44484
For Minors:
Parental or legal guardian consent is required for children who are not emancipated.
A consent form must be signed on-site by the parent or legal guardian.
A parent or legal guardian must accompany the minor to receive the vaccine.

Moderna COVID-19 Vaccine and Booster
The Moderna COVID-19 vaccine and booster are available at two ONE Health Ohio health centers for patients aged 6 months and older. Vaccinations are by appointment only at:

Moderna Booster:
Everyone aged 18 years and older is eligible for a Moderna booster shot. If unsure about receiving an additional dose, contact your healthcare provider for guidance. Authorization applies only to the Moderna COVID-19 Vaccine.

Prepare for Your Appointment
Vaccination Preparation Guide
Before arriving for your COVID-19 vaccination, ensure you have completed pre-registration. Bring your insurance card and a valid photo ID to your appointment. If you are uninsured, your vaccination will be covered through the HRSA program, ensuring no out-of-pocket costs. Please fill out the Vaccine Screening Questions and the Vaccination Consent Form prior to your visit. Remember, masks are required for those exhibiting symptoms, and hand sanitizers are available throughout our health centers.
