Please bring a valid photo ID, insurance card, and any medical records or medications you are currently taking. If you are applying for the sliding fee scale, bring proof of income (recent pay stubs, tax returns) and family size.

The Sliding Fee Scale (SFS) is a program that offers discounted charges for services to uninsured patients. It may also apply to patients with insurance who have high deductibles and/or copays, provided they qualify.

Is financial assistance available for dental services?
Yes, our sliding fee scale applies to both medical and dental services. Please complete the Sliding Fee Application Form to determine your eligibility for reduced fees.
How can I get a copy of my medical records?
How do I pay for services?
We accept cash, personal checks (with an imprinted name and address), and most major credit and debit cards. Please note that postdated checks are not accepted. To pay by credit card, visit your nearest ONE Health Ohio center or contact our billing department at 330-747-9551 ext. 10606.
How do I apply for the Sliding Fee discount?
