What It Means to Be a Federally Qualified Health Center (FQHC): The ONE Health Ohio Difference
At ONE Health Ohio, our mission has always been simple: to provide high-quality, affordable, and accessible healthcare for every person who walks through our doors. As a Federally Qualified Health Center (FQHC), we are proud to serve our communities with a model of care designed to remove barriers, support wellness, and create healthier futures for all.
But what exactly is an FQHC and what does it mean for our patients?

What Is a Federally Qualified Health Center (FQHC)?
FQHCs are community-based health organizations designated by the federal government to provide comprehensive primary care services in underserved areas. They are built on a foundation of accessibility, quality, and community impact.
To earn FQHC status, a health center must meet strict federal standards around quality of care, community governance, and access for all—regardless of insurance status or ability to pay.

How ONE Health Ohio Lives the FQHC Mission
1. Providing Affordable, Accessible Care
Whether a patient has insurance, is underinsured, or is uninsured, we ensure care remains within reach. Our sliding fee scale allows patients to receive high-quality services at a cost aligned with their income and family size.

2. Offering a Wide Range of Services
We aim to be a true one-stop resource for patient health. ONE Health Ohio offers comprehensive services including:
  • Primary medical care
  • Behavioral health services
  • Dental care
  • Pharmacy services
  • Women’s health
  • Pediatrics
  • Recovery services
  • Chronic disease management
  • Specialty programs like RISE Recovery, Menoblossom, Platinum Health, ONE Health FIT, Baby on Board, and more
This integrated approach means patients receive coordinated care under one trusted organization.

3. Expanding Access Through the 340B Drug Pricing Program
As an FQHC, ONE Health Ohio is eligible to participate in the 340B Program.  This is a federal initiative that allows health centers to provide medications to their patients at significantly discounted prices. These medications are available to ONE Health Ohio patients at our Prescription Pad Pharmacies, conveniently located inside of our Warren West and Falls Family Care locations.
This program benefits our patients by:
  • Lowering out-of-pocket costs for prescriptions
  • Supporting medication access for uninsured or underinsured patients
  • Allowing ONE Health to reinvest savings into expanded services, community programs, and improved patient care
The 340B program strengthens our ability to deliver affordable, comprehensive care, particularly for those who need it most.

4. Serving All Members of the Community
FQHCs are designed to serve medically underserved areas and populations. ONE Health Ohio proudly stands as a safety net provider for families, individuals, and communities who might otherwise struggle to access healthcare.

5. Community Governance
As part of federal FQHC requirements, our board of directors includes community members and patients. This ensures that our services genuinely reflect the needs and voices of the population we serve.

6. High Quality, Compassionate Care
FQHCs follow rigorous quality standards and participate in ongoing evaluations. Our providers and staff are committed to delivering patient-centered care rooted in respect, empathy, and evidence-based practices.

Why FQHC Status Matters for Our Patients
Being an FQHC allows ONE Health Ohio to offer:
  • Lower-cost services for every patient
  • Expanded access to care regardless of insurance
  • Reduced prescription costs through the 340B Program
  • Enhanced community programs and outreach
  • Coordinated, team-based care
  • Federal support that helps keep services strong and sustainable
In short, FQHC status empowers us to continue our mission of improving the health and wellness of the communities we serve—today and for years to come.

ONE Health Ohio: Caring for Our Community
For more than 40 years, ONE Health Ohio has been committed to making healthcare equitable, compassionate, and accessible. Being an FQHC is more than a designation, it’s a promise. A promise to serve, to support, and to put the needs of our patients and community first.
