STAY CONNECTED by enrolling in the ONE Health Ohio patient portal. You can communicate directly with your clinician, get 24/7 access to your health records, and request prescription refills. For more information on how to get enrolled in the ONE Health Ohio Patient Portal, email portal@onehealthohio.org. Please include your full name and date of birth when making requests.
If you have recently visited ONE Health Ohio and submitted your email, please check your email folder for a message from “no-reply@patientportal.net.” This email contains a link to set up your patient portal account.
