ONE Health Ohio was presented with the 2021
National Quality Leader Award: Behavioral Health from the Health Resources and Services
Administration (HRSA). This award is the Gold Medal Award for being one of the top quality
performers in the country and is given to only the top 1% of all health center organizations. ONE
Health Ohio was 1 of 17 organizations in the United States to receive this incredible honor.

ONE Health Ohio also received the Health Center Quality Leader Bronze Award, the COVID
Data Reporting Award, and the Patient Centered Medical Home badge!

with the team to create our programs.”

HRSA programs provide equitable health care to those who are economically, medically
vulnerable, and/or geographically isolated. Programs include participants who have HIV/AIDS,
are pregnant women, and unable to access high quality healthcare. In addition, HRSA also
supports the training of health professionals, assists in the distribution of providers to areas
where they are needed most, and improvements in the delivery of health care.

ONE Health Ohio is a Certified Patient-Centered Medical Home (PCMH) that provides
integrated healthcare services, such as medical, dental and behavioral health, across seven health
centers located in Youngstown, Warren, Newton Falls, Cortland and Alliance. RISE Recovery is
powered by ONE Health Ohio and offers Medication-Assisted Treatment to individuals who are
suffering from chemical dependency in Youngstown, Warren and Newton Falls. The mission of
